Would you hold on,
For all the days to come?
Or would you move on,
To someone new?
Would you remember me always,
Or would my memory slowly fade?
Would I still hold that special place in your heart,
Or would it be given to your new special someone?

You have posed a great thought. This poem could be taken in many different contexts. It could mean death, sickness, falling out of love, etc.
I liked your questions and if I knew the answers, my goodness that would be a powerful moment.
