Forgive mine eyes Oh, divine goddess. To thine great beauty They have been witness. For unworthy am I A lowly swain To admire such splendor Must ye disdain?
Forgive mine heart Ay, goddess lovely. Thy blissful charm Thy peaceful glory, For unworthy am I A swarthy swain To adore such beauty Must ye disdain?
Forgive this man before ye Oh, goddess lovely. Forgive mine eyes Forgive mine heart For loving thee reluctantly. Worthy am I not A craven swain For mine heart to beat only For thee, my love For thee! Ye must disdain! Ye must disdain!

This is a fine, fine poem you've written here. You use repetition with precision to reinforce the idea of 'the unrequited', as it were, and I empathised readily to the words . . .
"Forgive mine eyes
Forgive mine heart
For loving thee reluctantly. "
